- [x] Write `vitest.config.ts` using `createTestConfig()` from `@suaveplan/testing/config`

### 2. Error codes (`src/error-codes.ts`)

- [x] Define `StreamErrorCode` union type
- [x] Call `defineCodes` to register all `STREAM.*` codes
